    Clinical new patient enquiries

    If you’re ready to take the first step towards better mental health, you need to register on our clinical portal.

    Registration

    Please complete the following:

    1. Register on our clinical portal:

    2. Once registered we will email you an initial enquiry form and a consent form.

    3. When you have completed these forms, our clinicians will review your information and we will contact you to let you know if we can offer you an initial assessment.

    Please note:

    1. Dr L M Hayward Consultant Psychiatry Services offers outpatient services only and can only offer a service if you are over 18, resident in the UK and are registered with a UK GP.
